I, [Parent/Guardian's Full Name], on behalf of my minor child, [Participant's Full Name], hereby acknowledge and agree that participation in the sport of football involves inherent risks and dangers. In consideration of my child being allowed to participate in any and all football activities, including but not limited to practices, scrimmages, games, and related events (collectively referred to as the "Activities"), I voluntarily assume all risks associated with such participation.

 

I understand and acknowledge that football is a contact sport that carries the risk of physical injury, including but not limited to concussions, sprains, strains, fractures, dislocations, cuts, bruises, and other more serious injuries. I further understand that the risk of injury may arise from the actions or omissions of other participants, coaches, officials, or spectators, as well as from the condition of the playing area or equipment used.

 

In consideration of my child being permitted to participate in the Activities, I hereby release, waive, discharge, and covenant not to sue Fort McMurray Football Academy, its officers, directors, employees, volunteers, agents, and representatives (collectively referred to as the "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child or any property belonging to my child, whether caused by the negligence of the Released Parties or otherwise.

 

I understand that this Waiver of Liability and Assumption of Risk shall be binding upon me, my child, our heirs, executors, administrators, and legal representatives, and that we are relinquishing substantial legal rights by signing this document. I acknowledge that I have read and fully understand the terms and conditions herein, and I voluntarily sign this waiver without any inducement, assurance, or guarantee being made to me.

 

I affirm that my child is physically fit, has no medical conditions that would prevent their participation in the Activities, and has adequate medical insurance to cover any injury or damages that may occur as a result of their participation.

 

I hereby grant permission to the Released Parties to seek medical treatment for my child in the event of an emergency and agree to indemnify and hold harmless the Released Parties from any claims, demands, actions, or causes of action arising out of or related to any such medical treatment.

 

I have carefully read and fully understand the contents of this Waiver of Liability and Assumption of Risk and voluntarilly accept
