I hereby give my consent for Fort McMurray Football Academy/volunteers/associates to take photographs and videos of me/my child/children during the Registered events/courses/games. I understand that these photographs and videos may be used for promotional and marketing purposes, including but not limited to social media, website, and print materials.

 

I understand that my name/child/childrens name may be used in connection with the photographs and videos, but my contact information will not be shared without my permission.

 

I release Fort McMurray Football Academy, its employees, and any third parties involved in the creation or publication of the photographs and videos from any liability or claims arising from the use of the photographs and videos.

 

I acknowledge that I will not receive any compensation for the use of the photographs and videos.
